The short version: LASIK, SMILE, and PRK are all proven vision correction procedures available in Medellín — but not every clinic offers every option. The right procedure depends on your corneal anatomy, prescription, and lifestyle, not on marketing claims. This guide maps which technologies are available at Medellín clinics and helps you understand which procedure fits your eyes.
Three Procedures, Three Approaches
All three procedures correct refractive errors — myopia (nearsightedness), hyperopia (farsightedness), and astigmatism — by reshaping the cornea so light focuses properly on the retina. The difference is in the method of access.
LASIK creates a thin corneal flap using a femtosecond laser, reshapes the tissue underneath with an excimer laser, and replaces the flap. It has been performed since the mid-1990s and has the largest body of clinical evidence. Vision recovery is rapid — most patients see clearly within hours.
PRK removes the outer layer of the cornea (the epithelium) entirely, then reshapes the underlying tissue with the same excimer laser. No flap is created. The epithelium regrows over 3–5 days, which is why PRK recovery takes longer — functional vision returns in about a week, with full stabilisation over 1–3 months.
SMILE (Small Incision Lenticule Extraction) uses a femtosecond laser to create a disc-shaped piece of tissue (a lenticule) within the cornea, which the surgeon removes through a 2–4mm incision. No flap, no surface removal. It is the newest of the three, approved by the FDA in 2016.
Head-to-Head Comparison
| Factor | LASIK | PRK | SMILE |
|---|---|---|---|
| Corrects | Myopia, hyperopia, astigmatism | Myopia, hyperopia, astigmatism | Myopia, astigmatism only |
| Corneal access | Flap (20mm arc) | Surface removal | Small incision (2–4mm) |
| Visual recovery | Hours to next day | 5–7 days (functional), 1–3 months (full) | 1–3 days |
| Dry eye risk | Moderate (temporary) | Lower than LASIK | Lowest of three |
| Pain/discomfort | Minimal | Moderate for 2–3 days | Mild for 1–2 days |
| Corneal strength | Slightly reduced (flap) | Preserved | Well preserved |
| Price in Medellín | $1,400–$2,000 | $900–$1,300 | $2,000–$3,500 |
| Best for | Most patients | Thin corneas, contact sports, military | Dry eye prone, myopia patients |
Which Medellín Clinics Offer Which Technology
Not every Medellín ophthalmology practice has every laser platform. SMILE requires a specific Zeiss VisuMax or VISUMAX 800 femtosecond laser, which represents a multi-million-dollar investment. PRK and LASIK are more widely available because they use excimer laser platforms that most established clinics already own.
When evaluating a Medellín clinic, ask three specific questions about their technology:
- Which laser platform do you use? Look for current-generation equipment from Alcon (WaveLight EX500), VISX (Star S4 IR), Zeiss (MEL 90), or Schwind (Amaris). Avoid clinics that cannot name their equipment.
- Is wavefront-guided or topography-guided treatment available? These advanced mapping technologies create a personalised treatment plan based on your individual corneal topography — resulting in better outcomes than standard treatments.
- How many procedures of this type have you performed? Surgeon experience with a specific procedure matters more than the general reputation of the clinic. A surgeon who has performed 5,000 LASIK procedures but only 50 SMILE procedures is more experienced with LASIK.
The laser platform and surgeon experience matter more than the procedure name. A well-performed PRK with current technology will outperform a poorly executed SMILE every time.
Who Gets What: A Decision Framework
You are likely a LASIK candidate if:
Your prescription is stable, your corneas are of adequate thickness (≥500 microns), you have no significant dry eye, and you want the fastest recovery. LASIK is appropriate for roughly 85% of vision correction candidates.
PRK may be better if:
Your corneas are thinner than the LASIK threshold, you have an active lifestyle involving contact sports (boxing, martial arts, rugby), you are in the military or a profession where flap dislodgement is a concern, or you have basement membrane dystrophy. PRK produces the same final visual outcome as LASIK — the difference is entirely in the recovery period.
SMILE may be better if:
You are primarily myopic (nearsighted) with or without astigmatism, you are concerned about dry eye, or you prefer a minimally invasive approach. Note that SMILE currently cannot correct farsightedness — if you have hyperopia, LASIK or PRK are your options.
Do not choose a procedure based on marketing. The correct procedure is determined by your surgeon after a comprehensive eye examination, not by an advertisement or a website — including this one. Any surgeon who recommends a procedure before examining your eyes is a red flag.
The Real-World Recovery Difference
For international patients travelling to Medellín, recovery timeline directly impacts trip planning.
LASIK patients typically see 20/20 or close to it by the morning after surgery. You can walk around El Poblado, eat at restaurants, and enjoy the city by Day 2. Most are cleared to fly home by Day 5–7.
SMILE patients experience a slightly slower visual recovery than LASIK in the first 24 hours, but most achieve functional vision by Day 2–3. The same 5–7 day trip works well.
PRK patients need more time. Vision is blurry for the first 3–5 days as the epithelium regenerates. Plan for a 10–14 day stay if choosing PRK, and be aware that final vision continues improving for 1–3 months after the procedure.
If you are considering PRK, combine your recovery with an extended stay. Medellín's pleasant climate and low cost of living make a two-week recovery quite affordable — and far more enjoyable than recovering at home.
Price Is Not the Deciding Factor
PRK is the least expensive option at $900–$1,300 for both eyes. LASIK runs $1,400–$2,000. SMILE is the most expensive at $2,000–$3,500 — reflecting the costly equipment required. But the price difference between these procedures in Medellín is small compared to the difference between any of them and US pricing. Even SMILE in Medellín costs less than PRK in most American cities.
Choose the procedure your surgeon recommends based on your eye anatomy. The cost difference between a $1,400 LASIK and a $2,500 SMILE is negligible against a lifetime of glasses-free vision.
